1. Who we are
This website, oralhealthgovernance.com.au, is operated by Continuum Health Group Pty Ltd (ABN 34 698 399 058, ACN 698 399 058). The Oral Health Governance Toolkit is a product of Continuum Health Group. For privacy enquiries, contact info@oralhealthgovernance.com.au.
2. About this policy
This Privacy Policy applies to all personal information collected through this website. It is governed by the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). Personal Information means information or an opinion about an identified individual or an individual who is reasonably identifiable, whether the information is true or not and whether it is recorded in a material form or not. Sensitive Information, as defined in the Privacy Act, includes information about an individual's health, racial or ethnic origin, and other categories. We do not collect Sensitive Information through this website.
3. What information we collect
We collect personal information through this website when you choose to submit one of our forms. The forms and what each collects are:
Request a Toolkit overview. Your name, email address, organisation name, role, and sector.
Register your interest (waitlist). Your name, email address, organisation name, role, and sector; whether you have asked us to call you and, if so, your preferred time; the wording of the consent you agreed to; and whether you separately opted in to receive regulatory updates.
Download the Audit Readiness Checklist. The details you enter into the checklist request form, which are your name, email address, and organisation.
We do not collect health information, payment details, Medicare numbers, NDIS participant numbers, or any other sensitive information through this website.
4. How we collect your information
We collect personal information directly from you when you submit one of the forms described in section 3. We do not collect personal information about you from third parties through this website.
The overview and waitlist forms are hosted on this website and submit your details to our capture workflow (see section 6). The Audit Readiness Checklist request is collected through a form provided by Tally (tally.so), a third-party form service. This website uses cookies set by Cloudflare for security and performance purposes; these cookies do not identify you personally. You may disable cookies in your browser settings, though this may affect site functionality.
5. How we use your information
Request a Toolkit overview. We use these details solely to respond to your request and to send you the overview you asked for, and to answer follow-up questions about the Toolkit. We will not add you to a marketing list or send you direct marketing on the strength of an overview request. If you want to hear more from us, you can separately register your interest through the waitlist.
Register your interest (waitlist). If you register your interest, you are asking us to email you about the release of the Toolkit, and we use your details for that purpose. If you also opted in to regulatory updates, we use your details to send you those updates as well. You can unsubscribe from either at any time using the link in our emails or by contacting us.
Download the Audit Readiness Checklist. We use these details to deliver the checklist you requested and to answer follow-up questions about it. We will not send you direct marketing unless you separately register your interest.
6. How we store your information and who processes it
When you submit the overview or waitlist form, your details are sent to our capture workflow, which runs on the n8n automation platform hosted on our own server infrastructure in Australia (Amazon Web Services, Sydney region). The workflow records your submission, including the consent wording you agreed to, in a log we keep as our record that consent was given, and passes your contact details to Klaviyo, our email communications platform, so we can send the emails you have asked for.
We use the following third-party service providers to operate this website and to communicate with you. Each processes your information only for the purposes we specify:
Klaviyo — email communications and list management.
Tally — the Audit Readiness Checklist request form.
Cloudflare — website security, performance, and email routing.
n8n — the capture workflow that receives your form submission and records your consent. This runs on our own server infrastructure rather than a third party's platform.
Supabase — authentication for the member hub (hosted in the Sydney region). The hub is available only to organisations that have purchased the Toolkit and been sent an access invitation; there is no public sign-up. If you do not hold a hub account, Supabase does not hold information about you.
We implement appropriate security measures to protect your information from unauthorised access, misuse, or disclosure.
Retention. We retain the contact details you submit for as long as we are in contact with you about the Toolkit, and then for a maximum of two years after our last contact, after which they are deleted. Records of the consent you gave (the consent wording, the date, and the fact of consent) are kept for as long as we rely on that consent and for a reasonable period afterwards, so that we can demonstrate the consent was given. This is an exception to the two-year deletion above and is required so we can meet our obligations under the Spam Act 2003 (Cth) and the Privacy Act.
7. Overseas disclosure
Some of the service providers we use store or process information outside Australia. Under Australian Privacy Principle 8, we tell you where this is likely to happen.
Klaviyo is based in the United States, and your contact details and email engagement information are stored and processed there. Tally is based in the European Union (France) and processes the Audit Readiness Checklist request details there. Our capture workflow and consent log (n8n) and the member hub authentication (Supabase) are both hosted in Australia. By providing your information through this website, you consent to it being disclosed to the overseas providers named above on the basis described in this policy. Where we disclose personal information overseas, we take reasonable steps to ensure it is handled consistently with the Australian Privacy Principles.
8. Who we share your information with
We do not sell, rent, or share your personal information with any third party for their own commercial purposes. We share it only with the service providers listed in section 6, and only so they can perform the functions we use them for. We may also disclose personal information where required or authorised by law.
9. Your rights
Under Australian Privacy Principle 12, you have the right to access the personal information we hold about you. Under Australian Privacy Principle 13, you may request correction of inaccurate, incomplete, or outdated information. To exercise either of these rights, contact info@oralhealthgovernance.com.au. We will respond to all requests within 30 days. You may also withdraw your consent to our use of your information at any time, subject to our legal obligations, and you can unsubscribe from our emails at any time.
10. Complaints
If you believe we have handled your personal information in a way that does not comply with the Australian Privacy Principles, you may lodge a complaint with us at info@oralhealthgovernance.com.au. We will acknowledge your complaint within five business days and provide you with a reference number. We will respond in full within 30 days. If additional time is required, we will notify you in writing. If you are not satisfied with our response, you may lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or by calling 1300 363 992.
11. Changes to this policy
We may update this Privacy Policy from time to time. The current version will always be available at oralhealthgovernance.com.au/privacy.
